Alcohol and Delirium Tremens

Patients at greatest risk for death are those with extreme fever, fluid and electrolyte imbalance, or an intercurrent illness, such as occult trauma, pneumonia, hepatitis, pancreatitis, alcoholic ketoacidosis, or Wernicke-Korsakoff syndrome. Alcoholic individuals frequently have large total body deficits of magnesium, and symptoms and signs of magnesium deficiency include hyperactive reflexes, weakness, tremor, refractory hypokalemia, reversible hypoparathyroidism with hypocalcemia, and cardiac dysrhythmias. Thiamine can be useful for preventing Wernicke encephalopathy (confusion, ataxia, ophthalmoplegia), an acute disorder caused by thiamine deficiency, and Korsakoff syndrome (memory impairment, amnesia), a late manifestation of thiamine deficiency. As already mentioned, co-morbidity is the rule rather than exception in DT.

Alcohol consumption spans a spectrum ranging from low risk to severe alcohol use disorder (AUD). AWS represents a potentially life-threatening medical condition typically affecting AUD patients abruptly decreasing or stopping alcohol consumption. AWS should be considered in the differential diagnosis of any patients showing symptoms of autonomic hyperactivity. The use of a clinician-administered scale (CIWA-Ar or Alcohol Withdrawal Scale) is important to diagnose AWS and start adequate treatment. BZDs represent the gold standard treatment as a result both for their high rate of efficacy and being the only medications with proven ability to prevent the complicated forms of AWS (seizures, DTs).

Hence thorough clinical examination and laboratory investigations must be carried out for patients with DT. Fluid and electrolyte imbalance and nutritional issues should be taken care of. Intravenous fluid should be used cautiously because of the possibility of volume overload but can be useful in patients with excessive sweating, hyperthermia, and vomiting. A “fixed-dose”, rather than a “loading dose” or a “symptoms-triggered” regimen can be adopted for the management of AWS.
